NEW -- December 9, 2003 -- Click here to view and download an adobe acrobat version of the CLAIMS ADMINISTRATOR'S INITIAL LETTER TO CLAIMANTS. This letter to claimants introduces the Claims Administrator Edgar Gentle, explains the $500.00 advance procedure, and references the Claimant Advance Payment Registration Form. Instructions to claimants on receiving advance and other payments are also provided. In addition, a calendar of times for meetings in Anniston to discuss claims issues is included.

This form must be completed by claimants and returned to the Claims Administrator no later than March 31, 2004, in order for Claimants to receive advance and future payments under the settlement.

The Anniston Quality Healthcare Clinic participates in Pfizer's Sharing the Care program. This program donates most of Pfizer’s leading prescription medicines to more than 380 community health centers across the country, including the Anniston clinic, to help their low-income uninsured patients.

Click on the picture of the card to the right to get more information about Pfizer's Share Card program. Qualified participants in the program can receive substantial discounts on Pfizer drugs. If you’re on Medicare, you may qualify to pay only $15 for each Pfizer drug. Some limitations apply. See the Pfizer website for details and restrictions. The card also entitles members to free information on up to 16 different health topics, and members can speak with someone about program questions, anytime.
